A story-driven game that returns to the roots of the world-famous Mafia series and takes players into the Sicilian underworld at the beginning of the 20th century. The main character, Enzo Favara, tries to make his way in the world of organised crime, where he must fight for survival and prove his loyalty. The game combines cinematic storytelling, an authentic historical atmosphere, and intense action.
Players can travel on foot, on horseback, or in period automobiles through vineyards, ruins, crypts, and grand opera houses.
A single building in the game can contain more than one million polygons, compared with approximately 20,000 to 30,000 in similar buildings in previous Mafia games, allowing for significantly more detailed architecture and environments.

Hangar 13 is part of the 2K Games family of development studios and consists of teams working across several locations. Most of its developers are based in Brno and Prague (Czech Republic), with another team in Brighton (United Kingdom). The studio, which includes veterans who worked on the first two games in the series, Mafia: The City of Lost Heaven (2002) and Mafia II (2010), debuted with Mafia III (2016). It later returned to the series’ roots with the successful remake Mafia: Definitive Edition (2020), followed by the sports title TopSpin 2K25 (2024) and, most recently, Mafia: The Old Country (2025).
